- Cancellations or rescheduling of appointments require minimum of two (2) business days' notice, excluding weekends and holidays.
$150
- +Second or more
- Must be paid before you will be rescheduled.
- May be discharged after 3rd no show or late cancellation in one year
$200/hr
- +Add'l Services
- Charges may apply for refills, completion of forms & contact requiring clinical decisions between appointments.
